Hepatitis C Case Management & Clinic Capacity Building

Building on our Hepatitis C (HCV) ECHO series, our HCV Case Management & Clinic Capacity Building series dives more deeply into the environmental, systemic, and psychosocial factors that can make HCV complex to manage well. Patients living with HCV may face a number of barriers—from trouble adhering to treatment to difficulty navigating their health insurance—that must be addressed to make treatment as effective as possible. Managing patients with Hepatitis C (HCV) and obtaining HCV medications for patients requires an HCV program with a tracking mechanism as well as managing the prior authorization process for most public health insurance plans in the state of Illinois. This causes an undue burden on primary care practices where providers are prepared to treat HCV but still face obstacles doing so. The ECHO-Chicago curriculum in HCV Case Management & Clinic Capacity Building is designed to expand community health center capacity for holistically managing patients with HCV. This training series provides best practice recommendations for strengthening existing HCV programming and provides health centers with a toolkit to assist in the creation of an HCV workflow individualized to each participating clinic. We will cover topics including treatment readiness and monitoring, program evaluation, and insurance navigation. This series is suitable for primary care providers and other health center staff involved in programming and treatment for patients.
